  • End User is a consumer
  • Error is a debit Unauthorized Entry or Entry Not in Accordance with Authorization
  • Entry is a debit Entry using one of the following SEC Codes: CCD or CTX
Initiate by the 60th calendar day after the date of the original Entry

Debit Authorization Revoked by Consumer
  • End User is a consumer
  • Error is a debit Entry with Revoked Authorization
  • Entry is a debit Entry using one of the following SEC Codes: PPD, POS, SHR, TEL, WEB, or IAT
Initiate by the 60th calendar day after the date of the original Entry

Payment Stopped
  • End User is a consumer or business
  • Error is that a debit Entry was processed despite being the subject of a valid Stop Payment Order
  • Entry is any debit Entry that is the subject of a valid Stop Payment Order
Initiate by the 2nd Business Day after the date of the original Entry

Other Unauthorized Debit
  • End User is a consumer or business
  • Error is an Unauthorized Entry
  • Entry is a credit or debit Entry using one of the following SEC Codes:
    – ARC, BOC, IAT, POP, POS, PPD, SHR, TEL, or WEB, (if to a consumer Account); or
    – ARC, BOC, IAT, or POP (if to a business Account)
  • Additional requirements:
    – End User warrants in writing that the signature on the source document is not authentic, valid, or authorized (if ARC or BOC Entry)
    – End User warrants in writing that the signature on the written authorization collected by the Originator is not authentic, valid, or authorized (if POP Entry)
Initiate by the 60th calendar day after the date of the original Entry

Debit Not in Accordance with Authorization
  • End User is a consumer or business
  • Error is an Entry Not in Accordance with Authorization
  • Entry is a credit or debit Entry using one of the following SEC Codes:
    – IAT, PPD, POS, SHR, TEL, or WEB (if to a consumer Account); or
    – ARC, BOC, IAT, POP, POS, PPD, SHR, TEL, or WEB, (if to a business Account)
Initiate by the 60th calendar day after the date of the original Entry

Entry Contains Invalid Account Number or Reversing Entry is Improperly Originated
  • End User is a consumer or business
  • Error is that either
    – the Entry contains an invalid Account Number and Customer suspects it was initiated under questionable circumstances; or
    – Customer or the End User identified a Reversing Entry that was improperly initiated by the Originator or ODFI.
  • Entry is any debit or credit Entry or Reversing Entry
Initiate by the 2nd Business Day after the date of the original Entry

Originator Identification Number is Incorrect
  • End User is a consumer or business
  • Error is that the Originator identification number used to allow the End User to identify the Originator’s account is incorrect
  • Entry is a debit or credit Entry using one of the following SEC Codes: CIE or MTE
Initiate by the 2nd Business Day after the date of the original Entry

Credit Entry Refused by End User
  • End User is a consumer or business
  • Error is that the End User is refusing to accept a credit Entry from the Originator for any reason
  • Entry is any credit Entry
Initiate by the 2nd Business Day after the date of receipt of the End User’s communication

Unauthorized Debit to Business Account
  • End User is a business
  • Error is an Unauthorized Entry
  • Entry is a debit Entry using one of the following SEC Codes:
    – CCD, CTX, POS, PPD, SHR, TEL, WEB
Initiate by the 2nd Business Day after the date of the original Entry
